Fever is one of the most common symptoms we came across in our daily life. It is mistakenly taken as a disease but it is an indication of an underlying infectious ailment. It is characterized by an increase

in body temperature. Other names for fever are hyperpyrexia, hyperthermia, and raised Body temperature. Everyone has to cope with it many times in his/her life. Mild Fever may get resolved automatically but in severe cases, extensive workup should be done to reach the definitive diagnosis.

Thermoregulatory Centre

In the brain, there is a thermoregulatory sensor called the hypothalamus that controls our body temperature. Normally the body temperature is 98.6F but it does fluctuate according to our body metabolism, our body health and diurnal variations.

In general, a baby has a fever when their body temperature exceeds 100.4°F (38°C). A child has a fever when their temperature exceeds 99.5°F (37.5°C). An adult has a fever when their temperature exceeds 99–99.5°F (37.2–37.5°C).

Causes of Fever

Fever happens owing to a disturbance in the set point of temperature in the hypothalamus. This is due to

Viral and bacterial infections in the body

Maybe the side effects of various vaccines

Some children during their growing ages can develop fever,no need to worry about it is physiological

It can be due to inflammation of organs in the body like joint inflammation, heart and lungs inflammation, etc

Sometimes, excessive sunbathing can increase the body temperature. It is lethal as it can lead to heatstroke

Like vaccines, some drugs can also cause an increase in body temperature. Discontinuation of the drug can settle the temperature

As Fever is a symptom, not a disease, it is associated with a couple of other signs like shivering, dehydration, and disturbances in the blood pressure

Diagnosis of Fever

It demands extensive workup. Following tests should done to rule out the cause of fever

Complete Blood Examination will reveal an increase in the number of white blood cells due to infection.

Inflammatory markers are also very important in the diagnosis of fever. We can use the two most important markers of inflammation. These are the Erythrocyte sedimentation rate and C reactive Protein. Fever is caused by infection and ESR is raised in chronic while C Reactive protein is raised in acute infections. These two are the most sensitive makers and raised in every infection.

Liver Function and Renal function tests should be carried out to check the inflammation of the liver and kidneys.

Some other tests should be done to rule out the cause of fever. These include Dengue PCR Testing, CoronaVirus PCR Testing, Typhoid testing, and Malaria Tests.

Moreover, malaria is diagnosed by doing thick and thin-film tests, Coronavirus is confirmed by the PCR although Antibody testing is also present, Typhoid can be diagnosed on Widal test, typhidot test but the blood culture remains the test of choice for it

Treatment of fever

Fever is commonly managed at home with the help of water sponging. It will help in lowering the body temperature. Taking plenty of fluids and shower in the cold water can reduce the incidence of fever to a great turn.

Antipyretics are the medicines that are used to lower the body temperature. Commonly used antipyretics are acetaminophen, Paracetamol, indomethacin, etc. These medicines should be commenced after the proper water sponging.

Paracetamol infusions are mostly used in hospitals for the treatment of fever. They should be given when the temperature of the patient is between 100-101F.

Prevention of fever
